Discharge planning and aftercare services
Leaving rehab can be a difficult transition, especially when going from 24/7 monitoring and care to independent living. To help better navigate, we offer aftercare services that provide ongoing assistance to help overcome obstacles and avoid relapse. During residential treatment, you will explore the underlying causes and conditions of your substance abuse and learn healthy coping mechanisms to maintain wellness. Aftercare allows you to practice what you’ve learned in rehab in real-life situations while still obtaining professional guidance and support.
It’s essential to understand that recovery is a lifelong journey. While leaving rehab can be a significant step towards a better life, it can also be a vulnerable time. Although relapse is a common occurrence, it doesn’t mean that treatment has failed. Rather, it suggests that treatment and aftercare methods need to be tailored to meet your needs. Developing an individualized aftercare plan that addresses your overall wellness is an excellent way to prevent future relapses and get back on track after a setback. Remember that with the right support and strategies in place, you can achieve lasting recovery.
Solutions that Work
PHP
After you complete residential treatment, transitioning to a partial hospitalization program (PHP) is the most recommended method for obtaining long-term sobriety. During PHP, you’ll participate in structured sessions and therapy groups that are similar to what you experienced during detox. These sessions can help you further develop coping mechanisms, learn essential life skills, and provide ongoing support to help maintain your sobriety. Living in a sober community can provide additional encouragement, support, and accountability to help reinforce healthy habits and maintain your progress. However, it’s important to note that in most cases, you’d need permission to leave the facility or house to ensure you are not put in situations that could trigger a relapse
IOP
Intensive outpatient programming, better known as IOP, is the next step in the continuum of care after PHP. IOP offers a more flexible treatment option, with patients spending just 10-12 hours a week attending therapy and counseling sessions. The lower time commitment allows patients to explore a new level of independence while still having access to the valuable support needed to maintain wellness. This level of care focuses heavily on relapse prevention and life-skills training to equip patients with the tools they need to navigate potential triggers and challenges. By working with a therapist or counselor, patients can address issues that arise in real-time and gain the necessary support to stay on the right track.
Sober Living
Looking for a safe and sober place to stay after rehab? Whether you’re in PHP, IOP, or a standard outpatient program, a sober living home in West Palm Beach can be a great choice for aftercare. One of the leading contributors to relapse is the absence of stable, supportive, and sober housing. A lot of people who complete treatment do not have a supportive or substance-free living environment to return to, or they can’t afford living expenses on their own. Thankfully, sober living homes exist to help individuals transition from rehab back to the real world while maintaining a sense of safety, support and sobriety.
As a resident of a sober living home, you will be required to undergo random drug and alcohol testing, follow house rules, pay your rent, and attend sober support groups. You will also have the opportunity to share your living space with others who also value sobriety and a healthy lifestyle. While sober living homes aren’t a substitute for professional treatment, they can help you develop structure and accountability in your daily routine, which is essential for maintaining recovery.
